It is expected that deductibles, co-payments, and other amounts not covered by insurance will be paid prior to services being provided. Accepted forms of payment include personal check and credit card. Payment may be made over the phone prior to your procedure or at check in at Crown Point Surgery Center. If you have questions at any time, let us know.

Questions About Your Bill

After you’re discharged, a statement will be mailed to you for any balance due after your insurance company has processed your claim. Payment is due upon receipt and may be made by personal check or credit card.

When you receive your statement, please contact the phone number printed on your statement.

If you have health insurance coverage, you are strongly encouraged to consult with your health insurer to determine accurate information about your financial responsibility for a particular health care service provided by a health care provider associated with Crown Point Surgery Center. If you do not have health insurance coverage, you are strongly encouraged to contact our business office personnel at (720) 974-6499 to discuss payment options prior to receiving a health care service from a health care provider at this office since posted health care prices may not reflect the actual amount of your financial responsibility. Actual services provided during a surgical procedure may vary from the scheduled procedure and price quote, including but not limited to the medically necessary use of high cost drugs, implants, supplies and any procedures other than the original quote based on individual circumstances for each patient case.
